[odf-discuss] MS OOXML fast track and British Standards

Has anyone got a filled in version of the form below I could asapt
rather than writing everything from scratch? I want to make a submission
form a UK government approved awarding body specialising in education
about open standards ;-) Also I can pass it on to BECTA and hope they
make a submission too.
